I accept all major credit cards, including HSA and FSA credit cards. At the time of completing intake forms, I will ask that you input a credit card to keep on file in my secure portal. At the end of each session, your card will be charged. Sessions are not held until paperwork and card information is completed.

Cancellation Policy

I require at least 24 hours advance notification for appointment cancellations and rescheduling requests. You will be responsible for the entire fee (self private pay rate-see above) if you provide less than 24 hours notice and the credit card on file will be charged. No Surprises Act and Good Faith Estimate (GFE) Information

As of January 1, 2022, all healthcare providers are required to provide estimates for the costs of your care. The GFE shows the cost of items and services that are reasonably expected for your healthcare needs and treatment. This will be provided by this office and/or as requested. The GFE does not include unexpected costs that could arise during treatment.

Right to Receive a Good Faith Estimate of Expected Charges

YOU HAVE THE RIGHT TO RECEIVE A “GOOD FAITH ESTIMATE” EXPLAINING HOW MUCH YOUR MEDICAL CARE WILL COST.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

-You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.-Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

-If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.
